  • People with diabetes: Nerve damage can dull pain signals, which makes a heart attack harder to spot when it happens.

  • Older adults: Symptoms tend to be milder or less typical with age, so a heart attack can pass unnoticed

  • People with high blood pressure or high cholesterol: These conditions accelerate coronary artery disease and increase the likelihood of plaque rupture and arterial blockage

  • History of prior heart attack: Prior infarction increases the risk of subsequent events, some of which may be silent

  • Smokers: Smoking damages blood vessels and sharply raises the risk of a silent heart attack.

Silent Heart Attack Symptoms in Women vs Men

Women are more likely to experience silent or atypical heart attacks, and their presentations differ in ways that contribute to delayed diagnosis.

Men more commonly experience the classic pattern: pressure or crushing chest pain radiating to the left arm, jaw or shoulder, accompanied by sweating and shortness of breath. Even when mild, this pattern is more recognisable.

Women are more likely to present with fatigue, nausea, jaw or neck discomfort, back pain, breathlessness and sleep disturbances rather than chest pain.   • Quit tobacco: Stopping smoking reduces the excess risk of coronary artery disease within one to two years. 

  • Maintain a healthy weight and diet: A diet based on whole grains, vegetables, legumes (dal, rajma, chana) and healthy fats such as mustard oil and minimal processed food reduces cardiovascular risk across Indian dietary patterns.

  • Exercise regularly: 150 minutes of moderate-intensity aerobic activity per week, such as brisk walking, reduces cardiovascular risk and supports blood pressure, blood sugar and weight control.
