Which Bacteria Does Amoxicillin Fight
Amoxicillin is active against gram-positive bacteria including Streptococcus pneumoniae, Streptococcus pyogenes, Enterococcus faecalis and Staphylococcus epidermidis and gram-negative bacteria including non-beta-lactamase-producing Haemophilus influenzae, Escherichia coli, Proteus mirabilis and Helicobacter pylori (in combination regimens).
Amoxicillin is not effective against beta-lactamase-producing bacteria - organisms that produce an enzyme destroying the beta-lactam ring before it can act. MRSA, most hospital-acquired gram-negative bacteria and Pseudomonas aeruginosa are not susceptible. When beta-lactamase-producing organisms are suspected, amoxicillin-clavulanate is used.
Infections Amoxicillin Is Commonly Prescribed For
They are:
Streptococcal pharyngitis (strep throat): First-line treatment, typically 500mg three times daily for 10 days
Acute otitis media (middle ear infection): First-line in children and adults
Community acquired pneumonia: Mild to moderate cases as outpatient treatment
Sinusitis: Bacterial sinusitis persisting beyond 10 days or with severe symptoms
Urinary tract infections: Uncomplicated UTI caused by susceptible E. coli and Proteus mirabilis
Dental infections: Periapical abscess and dental prophylaxis before procedures in high risk cardiac patients
Skin and soft tissue infections: Mild infections caused by susceptible Streptococcus and Staphylococcus species
H. pylori eradication: Part of triple or quadruple therapy regimens for peptic ulcer disease and gastritis.
Tablets vs Capsules vs Oral Suspension - Which Form Is Used When
Amoxicillin is available as 250mg and 500mg capsules, 500mg and 875mg tablets, and as oral suspension (125mg/5mL or 250mg/5mL) for children or those who cannot swallow solid forms. The oral suspension is reconstituted with water, stored refrigerated once prepared, discarded after 14 days and shaken before each use.
The 875mg tablet form is primarily the amoxicillin-clavulanate (Augmentin) formulation. Parenteral amoxicillin is used in hospitalised patients with severe infections where oral administration is not feasible.
How Amoxicillin Works
Amoxicillin is bactericidal it kills bacteria rather than merely inhibiting growth. It binds irreversibly to penicillin-binding proteins (PBPs), the enzymes responsible for the final stages of peptidoglycan synthesis. Blocking peptidoglycan synthesis causes the cell wall to become structurally defective.
This mechanism is time-dependent: the duration above the minimum inhibitory concentration (MIC) drives efficacy not the peak drug level. This is why amoxicillin is dosed multiple times daily rather than in a single large dose.
Common and Rare Side Effects
Common side effects are:
Nausea
Diarrhoea
Vomiting
Abdominal discomfort
A maculopapular skin rash.
Rare but serious adverse effects include:
Anaphylaxis
Angioedema
Clostridioides difficile-associated diarrhoea
Stevens-Johnson syndrome
Hepatocellular toxicity.
Amoxicillin Allergy - Signs and What to Do
True penicillin allergy occurs in a small amount of the population, though genuine confirmed allergy is less common - many patients have non-immune adverse reactions or have lost sensitisation over time. Immediate hypersensitivity reactions like urticaria, angioedema, bronchospasm or anaphylaxis typically occur within one hour and require immediate discontinuation and emergency treatment.
Delayed maculopapular rash appearing 72 hours or more after starting the course is usually non-IgE-mediated and may not represent true allergy. Any reaction during an amoxicillin course should be reported to the doctor.

Special Populations: Children, Pregnant Women, Elderly
Children: Amoxicillin is the most commonly prescribed antibiotic in paediatric practice. Dosing is weight-based typically 25 to 50mg/kg/day in divided doses via oral suspension for otitis media, pharyngitis and pneumonia. It is safe across all paediatric age groups when dosed correctly.
Pregnant women: Amoxicillin is a WHO and FOGSI-recognised safe antibiotic (Pregnancy Category B), used for Group B Streptococcus prophylaxis, UTIs in pregnancy and antepartum prophylaxis.
Elderly: Dose adjustment for renal impairment is important, as GFR may be significantly reduced even with apparently normal serum creatinine.
Antibiotic Resistance: Why Completing the Course Matters
Incomplete antibiotic courses are a major driver of resistance. When amoxicillin is stopped early, surviving bacteria are those that tolerated it longest - the most resistant. These multiply in the absence of competition and the next infection may be caused by a strain substantially harder to treat.
In India, ICMR surveillance consistently shows rising resistance to commonly used antibiotics including amoxicillin, driven by overuse, self prescription and incomplete courses. The full prescribed course whether 5, 7 or 10 days must be completed even if symptoms resolve before the last dose.
Amoxicillin vs Amoxicillin-Clavulanate (Augmentin) - What's the Difference?
Amoxicillin-clavulanate combines amoxicillin with clavulanic acid - a beta-lactamase inhibitor. Clavulanic acid has no antibacterial activity; it irreversibly inactivates the beta-lactamase enzymes that would otherwise destroy amoxicillin. The combination extends amoxicillin's spectrum to include beta-lactamase-producing Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis and several gram-negative organisms.
Amoxicillin-clavulanate carries a higher incidence of GI side effects particularly diarrhoea. It is used when beta-lactamase producing organisms are likely including complicated sinusitis, animal bites, diabetic foot infections or when initial amoxicillin has failed. Plain amoxicillin is preferred for infections where susceptibility is predictable.

Is amoxicillin safe to take without a prescription?
Amoxicillin requires a prescription in India (Schedule H drug). Self-prescribing risks treating the wrong organism, missing the correct diagnosis, selecting resistant strains and adverse effects. India's antibiotic resistance crisis is substantially driven by over the counter antibiotic use.
How quickly does amoxicillin start working?
Amoxicillin reaches therapeutic plasma concentrations within one to two hours. Symptom improvement is typically noticeable within 48 to 72 hours. Lack of improvement within this window needs immediate reassessment of the diagnosis and the appropriateness of amoxicillin.
Can amoxicillin treat viral infections like the flu or cold?
Amoxicillin has no antiviral activity. The common cold, influenza and most upper respiratory tract infections are caused by viruses. Prescribing amoxicillin for these provides no therapeutic benefit and contributes to antibiotic resistance. Antibiotics are appropriate only when a bacterial infection is confirmed or strongly clinically suspected.
What should I do if I miss a dose of amoxicillin?
Take the missed dose as soon as you remember unless the next scheduled dose is closer then skip the missed dose and resume the normal schedule. Do not double up doses. Maintaining consistent drug levels by dosing at regular intervals supports efficacy.
Is amoxicillin the same as penicillin?
Amoxicillin is a semi synthetic aminopenicillin (a member of the penicillin class). It is not the same as benzylpenicillin or phenoxymethylpenicillin but shares the same beta-lactam ring structure and mechanism of action. People with penicillin allergy should inform their prescriber before taking amoxicillin.
Can amoxicillin cause diarrhoea or stomach upset?
Yes in some patients. Taking amoxicillin with food or milk reduces nausea without significantly affecting absorption. Severe or persistent diarrhoea particularly with blood or mucus, should be reported promptly.
Is amoxicillin safe during pregnancy?
Yes amoxicillin is one of the safest antibiotics in pregnancy (Pregnancy Category B). It is routinely used for UTIs, Group B Streptococcus prophylaxis and other bacterial infections in pregnancy. The prescribing doctor should confirm the indication and that no alternative is more appropriate.
Can amoxicillin be taken with food or milk?
Yes and it is advisable to do so. Amoxicillin absorption is not significantly affected by food or milk. Taking it with food reduces nausea and abdominal discomfort, improving tolerability and adherence.
What happens if I stop taking amoxicillin before finishing the course?
Stopping early does not mean the infection is cleared. Residual bacteria particularly the most resistant survive and may multiply, producing relapse with harder-to-treat organisms. The full prescribed course must be completed.
