Clarithromycin is a macrolide antibiotic - a class that stops bacteria from making essential proteins. It is one of the most prescribed antibiotics in India for respiratory & skin infections. Clarithromycin comes as standard tablets, extended-release tablets, and oral suspension. How Does Clarithromycin Work?

Clarithromycin binds to the 50S ribosomal subunit (the part of bacterial protein-making machinery) and blocks protein production. Without these proteins, bacteria cannot replicate. This makes it bacteriostatic as it halts growth rather than killing outright, letting the immune system clear the infection.

It concentrates inside white blood cells, which carry it to infected tissue which is why it works against intracellular bacteria like Legionella and Mycoplasma.

Uses of Clarithromycin

Clarithromycin treats a wide range of bacterial infections. Common approved uses are:

  • Community-acquired pneumonia, including atypical causes such as Mycoplasma pneumoniae

  • Acute bacterial exacerbations of chronic bronchitis and acute maxillary sinusitis

  • Pharyngitis and tonsillitis caused by Streptococcus pyogenes

  • Skin and soft-tissue infections like cellulitis, impetigo, folliculitis

  • H. pylori eradication as triple therapy with a proton pump inhibitor and amoxicillin

  • Disseminated MAC infection in HIV-positive patients; pertussis treatment and prophylaxis

Clarithromycin does not work against viral infections. Using it for viral illness accelerates resistance without benefit.

How & When to Take It

  • Standard tablets are taken twice daily, roughly 12 hours apart, with or without food. Extended-release tablets are taken once daily with the evening meal and food improves their absorption.

  • Swallow the tablet as a whole and do not crush it. Oral suspension must be shaken before each dose.

  • Complete the full course even if symptoms resolve early. Stopping 3–4 days in leaves residual bacteria alive and selects for resistant strains.

Side Effects of Clarithromycin

Gastrointestinal effects are the most common:

  • Nausea, abdominal discomfort, diarrhoea 

  • Clostridioides difficile-associated diarrhoea 

  • Clarithromycin prolongs the QT interval 

  • Headache and insomnia 

  • Rare and serious side effects:

  • Skin rash 

  • Liver enzyme elevation

  • Reversible hearing loss.

Can I Take Clarithromycin Daily?

Yes daily use is exactly how it is prescribed. Standard dosing is twice daily for 7–14 days. Long-term clarithromycin is prescribed for MAC prophylaxis in HIV-positive patients with CD4 counts below 50 cells/microlitre and for some non-tuberculous mycobacterial lung infections, with regular liver monitoring.

Precautions 

Clarithromycin requires care in specific groups:

  • Liver disease

  • Kidney impairment

  • Heart conditions

  • Myasthenia gravis

  • Pregnancy

  • Avoid grapefruit juice throughout the course as it inhibits CYP3A4 and raises clarithromycin levels unpredictably.

What If You Missed a Dose?

Take the missed dose as soon as you remember unless the next dose is due soon, in which case skip it. Never double up; two doses too close together raise the risk of QT prolongation and gastrointestinal side effects.

What If You Overdose?

Overdose causes intensification of normal side effects including severe nausea, vomiting, abdominal pain, and diarrhoea. No specific antidote exists. Activated charcoal may be given if the patient is alert and the overdose 

is recent. Seek emergency care immediately.

Caution With Other Drugs: Interactions

Key interactions are:

  • Statins like simvastatin and lovastatin

  • Warfarin

  • Colchicine

  • Digoxin

  • Carbamazepine

  • Phenytoin

  • Rifampicin

  • Rifabutin

  • QT-prolonging drugs like certain antipsychotics, antihistamines, and antiarrhythmics.

Dosage for Clarithromycin

Adult standard dosing:

  • Respiratory infections, pharyngitis, sinusitis: 250–500 mg twice daily for 7–14 days

  • Skin and soft-tissue infections: 250 mg twice daily for 7 days

  • H. pylori eradication: 500 mg twice daily for 7–14 days with a PPI and amoxicillin

  • MAC prophylaxis: 500 mg twice daily, long-term

  • Paediatric dosing: 7.5 mg/kg twice daily (max 500 mg per dose); not for infants under 6 months. 

Clarithromycin vs Azithromycin

Both are macrolides with overlapping uses, but differ in important ways:

  • Dosing: Azithromycin is typically a 3–5 day once-daily course whereas clarithromycin runs 7–14 days twice daily

  • H. pylori: Clarithromycin is first-line in eradication regimens

  • Drug interactions: Clarithromycin inhibits CYP3A4 significantly whereas azithromycin does not, making azithromycin safer alongside statins, warfarin, or colchicine

  • QT prolongation: Both carry cardiac risk but the interaction burden is higher with clarithromycin

  • Resistance: H. pylori clarithromycin resistance is rising in India
